Order forms will be available for pick up at the Middle School office, or you can download the form below, starting 11/26/2019.

End Date:  Friday, 12/6/2019
Please submit payment and Submission Form to the middle school office by end of day, Friday, 12/06/2019. Late submissions may not be processed.

Pick Up Date & Time:  Thursday, December 12th 6:30pm - 8:00pm
Orders may be picked up in the Middle School Cafeteria.
